This section establishes a 10-year demonstration program, referred to as the Program, to allow Medicare beneficiaries to apply their benefits to health insurance systems in other countries. The Secretary is tasked with designing the Program to evaluate its impact on the quality of care, cost of care to the government, reunification with family members, and strain on the U.S. medical infrastructure. Stakeholders, including the Department of State, clinical experts, beneficiaries, and foreign health insurance experts, will be consulted in the Program's design. Initially, the Program will include at least 11 countries, such as Canada, Germany, India, Israel, Korea, the Philippines, Panama, Mexico, Dominican Republic, and Ghana. Applicable Beneficiaries can voluntarily participate and terminate participation at any time, triggering a Special Election Period. The Secretary may cap the number of participants at no fewer than 150,000. Payments will be made to beneficiaries for healthcare costs in Selected Countries, up to the amount Medicare would pay for equivalent services in the U.S. The Secretary may administer the Program through Medicare Advantage Organizations. Annual evaluations will be conducted to determine the Program's effectiveness and inform recommendations to Congress.
