This section amends the Patient Protection and Affordable Care Act to establish a 'public health insurance option'. The Secretary is required to establish and offer the public health insurance option through the Exchanges, beginning on or after January 1, 2027. The Secretary's primary responsibility is to create an affordable health plan without compromising quality or access to care. The public health insurance option will be offered exclusively through the Exchanges and will offer bronze, silver, and gold plans. The Secretary may enter into contracts to perform administrative functions related to the option. States may establish advisory councils to provide recommendations on the public health insurance option's operations and policies. The Secretary shall establish geographically adjusted premium rates for the option, sufficient to cover costs and administrative expenses. Health care provider reimbursement rates will initially be negotiated, but if an agreement cannot be reached, rates will be based on Medicare's fee-for-service program. Start-up funding is authorized for the public health insurance option, to be repaid over a 10-year period.
